New: Ability to open a database of sets from macros and external clients

Added Qdv.UserApi.QdvManager.SetDatabaseFactory property to the user API. This allows for programmatic creation of set database objects from macros or external clients.

Bug Fix: Old contextual menus were removed in the reports generator

By mistake, we removed some contextual menus from the report generator at the time we implemented the WBS fields manager. However, if these menus are no longer needed in the WBS they are in the report generator to insert columns.
